Cumberland Valley initiative connects older adults with coordinated physical, mental and social support services
Cumberland Valley Area Development District's FlourishCare is a community-centered healthcare initiative designed to improve access and quality of care for older adults in rural Kentucky. Partnering with the University of Louisville and utilizing the Trager Institute's FlourishCare Centers of Excellence model, this program emphasizes whole-person wellness by integrating physical health, mental health and social support. Its mission is to provide coordinated, compassionate and preventative care that helps individuals maintain independence and a higher quality of life.
A central component of Cumberland Valley FlourishCare is collaboration. The program connects local healthcare providers, community organizations and support services to create a unified care network. Through this network, older adults gain access to services such as health screenings, chronic disease management, caregiver support and behavioral health resources, all tailored to the unique needs of rural communities. The program also incorporates the evidence-based 4Ms of Age-Friendly Care: What Matters, Medication, Mentation and Mobility.
Cumberland Valley FlourishCare also promotes workforce development, which includes training in dementia care, trauma-informed care and age-friendly practices, equipping local healthcare professionals and community workers to meet the challenges of aging populations.
By linking clinical care with community support, the program strengthens regional health systems and enhances the quality of life for older adults across the Cumberland Valley.
